Creation Health Management, Inc. is a personal care services (PCS, PAS) healthcare provider based in the Houston area. We service over 20 counties with personalized care plans tailored to every client’s unique needs. We work closely with seniors, those with chronic conditions like Alzheimer’s and Parkinson’s, and those recovering from surgery or illness.
Our single greatest goal is to maximize effective individual outcomes while minimizing costs and continuing to deliver high-quality patient care. We accept Medicaid, HMO, Texas Children, PCS, PAS, FC, private pay, and private insurance. We also meet every client’s personal needs with utmost respect and integrity. Our seasoned team cooperates with their medical care providers and their families to provide a personalized approach that seamlessly fits their health needs. As a result of collaboration and compassionate care, our clients maintain their independence and dignity as much as possible.
